Missouri Health Connection Partners with MU Sinclair School of Nursing on Project to Reduce Re-hospitalizations of Nursing Home Residents

MHC to Provide the Key Technology for Nearly $15 Million Grant-Awarded Project to Improve Patient Care, Lower Health Care Costs;

One of Only Seven Grants Awarded in US

                COLUMBIA, Mo. – Missouri Health Connection today announced it was selected by The University of Missouri Sinclair School of Nursing to serve as its technical partner on a nearly $15 million grant from the U.S. Department of Health and Human Services Centers for Medicare and Medicaid Services (CMS).  The grant funds will be applied towards a project to reduce avoidable re-hospitalizations of nursing home residents.

 Specifically, MHC will partner with the MU Sinclair School of Nursing, CMS, state Medicaid programs, and Primaris to improve care for residents at 16 nursing facilities in the greater St. Louis area by increasing communication and coordination.  MHC will provide the information technology platform for the nursing homes to use to communicate residents’ health information electronically and securely with referring hospitals.

                  Results from the project could serve as a best practice for transforming senior care and reducing costs nationwide.

                 MHC’s technology for this project includes MHC Direct Secured Messaging and MHC Clinical Viewer.  MHC Direct Secured Messaging enables healthcare professionals to securely send and receive patient health care information, such as patient care summaries, electronically. The MHC Clinical Viewer will allow authorized healthcare professionals to securely view comprehensive clinical data of their patients returning from the hospital.
